Adult Social Worker Hospital Team

We are looking for an Adult Social Worker to join a Hospital Discharge Team.

THIS POST REQUIRES A SOCIAL WORK QUALIFICATION OF A MINIMUM 2 YEAR POST EXPERIENCE.

About the team

This team works in a busy environment supporting those on sit at the Hospital. The team is designed to those who have been admitted to Hospital, the social workers will put together care plans so that they can be discharged. This team works collaboratively with other services in the council for the best interest of the individual.

About you

You will be completing the necessary assessments in relation to Adult Social Care. A Social Work Degree/DipSW/CQSW within a minimum of 1 year post ASYE experience is required. Experience of working with 65+, Physical Disabilities, Learning Disabilities, Mental Health/ and or hospital.

• £34.00 per hour (PAYE payment options available also)
• There is an option to be paid £35.00 per hour Umbrella with £150 weekly allowance
