Adult's Social Worker

We are looking for a Social Worker to join a Maximising Independence Team.

This role requires a Social Work Qualification with a minimum of 2 years post qualified experience.

About the team

This is a fast paced team who work to ensure patients are safely discharged from hospital and have enough support as they need. Carrying out bedside assessments, arranging appropriate care packages for comprehensive discharges and advocating for service users and their independence is at the forefront of the teams motivation. Being able to ensure service users have access to necessary resources and external support at the earliest stages is key to the success of this role.

About you

A valid UK driving licence and car is preferred but not essential for this role. The successful candidate will have worked in a hospital team or short term assessment setting. A degree within Social Work (Degree/DipSW/CQSW) with a minimum of two years experience working in an Adult's Social Work environment is essential.
