Job description

We are looking for an Adult's Social Worker to join a Hospital Discharge and Neighbourhood Team.

This role requires a Social Work Qualification with a minimum of 2 years post qualified experience.

About the team

This is a fast paced team that work to ensure patients are safely discharged from hospital and have as much support leaving the hospital as they need. The team prioritises working with service users and their carers promote and maintain their dignity, choice and independence. Use of D2A procedures, completing mental capacity act assessment and dealing with safeguarding procedures are all day to day responsibilities in this role.

About you

The successful candidate will have worked in a hospital team or short term assessment setting. A degree within Social Work (Degree/DipSW/CQSW) with a minimum of two years experience working in an Adult's Social Work environment is essential. A valid UK driving licence and car is essential for this role.
