Continuing Healthcare Social Worker

Hertfordshire County Council are looking for a Social Worker to join their Continuing Healthcare Team. This is a Locum Social Work role based in Hemel Hempstead.

Your responsibilities as a Continuing Healthcare Social Worker:

• Complete CHC assessments and reviews
• Work within the CHC framework
• Complete Care Act Assessments and reviews
• Attend dispute resolution meetings and/or CCG panel
• Multi-disciplinary working
• Completion of Mental Capacity Assessments and best interest decisions
• Completion of safeguarding enquires where applicable
• Present cases to panel when necessary

Requirements of you as a Continuing Healthcare Social Worker:

• A recognised Social Work qualification (Degree/DipSW/CQSW)
• A valid Social Work England registration
• Have competent knowledge and experience of Multidisciplinary/ CHC work
• A valid UK driving licence and vehicle are essential for this role

Location: Hemel Hempstead, Hertfordshire

Umbrella pay rate: £32.00 per hour
